3.1 · 54 reviewsGood and clean nursing home. My neighbor was sent to this facility for therapy services after a surgery. I visited her a few times and found the staff and nurses quite friendly and professional.
Pruitt of Lithonia has great staff. The Admission Director (TRI )is really my reason why I would put my love one there. She really shows the compassion gift that GOD has given her. And I love the fact that she knows the patients and she knows the families. While I’m there I watch her and she interacts with the patients as if they are her family. If you had about 10 of her this place would be banging. I also enjoy the HR I don’t know her name but she just dances with the patients and interacts very well. The Activity lady is nice also. Job Well Done Guys
A spacious facility with excellent staff. Therapy and the environment has seen a nice upgrade. Very welcoming skilled nursing facility.

The payment portal never works and the staff doesn’t know how to assist. Called customer service and they don’t know. This is every single month I have to redo this process. Horrible! Do better!
It's really nice. The staff is friendly starting with the first person at the door she always pleasant. The building clean, secured and patients are well kept. No nursing house smells.
My 5 stars isn’t for the facility as a whole it’s special people there for my father and I know they truly care it’s not just a job to be honest they care for more than my father. Mrs. Souky I heard she’s away at the moment but I pray she comes back. The new DON Joan Bernard she is outstanding she isn’t just a person in her office she’s visible she interact my father has a habit I nodding on his index finger and thumb he has late stage Alzheimer’s one day I came there and she started talking to me about a plan to see what can be done about it, no one has acknowledged the fact that he does that but she is concerned and that means a lot to me. Zee she’s OT my father was admitted the same day she started so she witnessed my father walking in the facility now he doesn’t walk the therapy department has tried different things with him. Jut thinking the CNA’s use to put these brace on his legs they haven’t done it since Pruitt took over and the PT person has been gone. But I appreciate Zee. Tri it’s a lot she was the social worker when my father came in of course as always not knowing someone and trusting your love ones in their facility was a lot especially being overprotective of him she has always gave me that sense of relief when she’s there I know she’s admission now but when she do see my dad she acknowledges him whether he can speak back or not. Tri is the only person that I feel can get the job done, honestly Tri is a part of the operation she knows so much about that facility and I truly appreciate her. Karen she’s a Med tech she is OUTSTANDING, caring, loving she don’t look at the residents as just another number. When she’s there I have no worries about my father I come to visit him everyday. Gloria she’s the activities coordinator she is great if there is n activity going on and he’s in the hall she will take him. Whenever she see me she always ask am I ok. The lady that conducts Bingo she make sure when I’m there and dads up she let me know that something is going on. Ms. Marie she is so nice as well as Ms.Theresa sorry if I spelled it wrong. It’s a couple other nurse on the D E F hall that go over and beyond. Maurice he’s the laundry supervisor he is very helpful when there problem he jumps on it. I’m not sure if there is anyone else I’m missing I do apologize but keep up the good work the others should find something else to do cause caring for people is not your thing. Have a great one ✌🏽
I would not recommend this facility period. Some staff members are very neglectful and lazy. Please don’t entrust your loved one to this place. I regret that I put my loved one there, they did not take good care of him.
I am extremely disappointed and frustrated with the experience my father had at your facility — especially after what felt like a promising start. During the admission interview, the director did a phenomenal job of selling the rehab center — highlighting the amenities, activities, and promising that my loved one would have a private room, even though I had initially requested a double occupancy. The room we were shown was clean, well-kept, and gave me some much-needed peace of mind. But the reality was a completely different story. When I returned to the facility the evening my father was transferred, I was horrified by the condition of the room he was placed in. The stench in the room was unbearable, the bed was filthy, and I had to use paper towels to clean it myself. The bathroom was disgusting — the commode seat had stained, dried feces sitting on top of it. How is this acceptable for any patient, let alone one who is vulnerable and in need of care? A nurse was notified, but it still took a member of housekeeping to finally clean the room — something that should have been done well before any patient was admitted. To make matters worse, I returned one afternoon to find my father still wearing the same clothes from the day before. That is neglect — plain and simple. To the staff members who did show attentiveness and compassion — thank you. But unfortunately, your efforts were overshadowed by serious lapses in basic cleanliness, care, and communication. As if things weren’t bad enough, the discharge process was handled in the most inconsiderate and unprofessional way possible. I was told, two days in advance at 4:30 PM while driving home from work, that my father had to be out of the room by midnight. That kind of last-minute notice is completely unacceptable. Traditions needs to do MUCH better — not just in selling a polished image, but in truly providing the care and respect that every patient and their family deserves.
