My Mother's in this facility for rehabilitation after leaving an inpatient facility. Inspite of needing some improvement. The way the STAFF (ALL OF YOU) treats my Mom like the queen she is. All the Staff are friendly, caring, attentative, loving, good spirited folk. I see such a big improvement with my Mother's overall look. She's thriving. Im just so amazed and thankful to you guys for taking such good care of my Mother. Especially, me needing to leave her there for 3 weeks while taking care of her business, in another state. I do see improvement in her and I know she still has along way to go with her PT, OT and ST with more time, she'll get there. I've seen some of the comments on here. You cannot just put your relatives some where and are a "no show" and expect so much. Handle your business. Show up! Thank you LCCNR for your amazing work!!

My family had an extremely disappointing experience at this facility, and I would not recommend it to anyone. My uncle was admitted on Monday, July 6. When I visited him the following morning, the first thing I noticed was the lack of professionalism. I walked onto the unit, said hello, and although about five staff members were sitting at the nursing station, no one acknowledged me. When I got to my uncle’s room, it was dirty. The curtains were stained, the room did not appear clean, and the bed remote did not work. Before we arrived, my cousin had spoken with the social worker, who said…

Before we arrived, my cousin had spoken with the social worker, who said they would conduct a 72-hour care plan meeting. He knew we were on our way and said the meeting could take place once we arrived. When we entered the meeting room, the social worker, MDS coordinator, and unit manager were already there. Instead of reviewing my uncle’s care plan, the social worker simply asked, “What questions do you all have?” We were shocked. A 72-hour care plan meeting should be organized and focused on explaining the patient’s plan of care, goals, medications, therapy, and answering questions—not expecting the family to lead the meeting. After that experience, we decided we wanted my uncle transferred to another facility. I called the marketer to explain our concerns and let her know we wanted him moved. No one from the facility ever followed up, apologized, or even asked what had happened. There was absolutely no concern shown for our experience. When we returned to pick him up, the social worker walked into the room, handed us a brown envelope, and walked right back out without saying a single word. We packed his belongings and waited, but no one came to assist us. I finally had to go to the nursing station to find out who his nurse was and request a wheelchair for his transfer. The nurse eventually came in, disconnected his feeding tube, and handed us a bag containing his medications and paperwork. While disconnecting the feeding tube, we noticed his pants were soiled, so a nursing assistant came in to change him. During the brief change, my uncle suddenly began having a seizure. The nursing assistant immediately called for the nurse. The nurse came to the doorway, looked at him, and walked away. She never returned to assess him or provide care. My cousin went to find the nurse and told her that my uncle needed to go to the hospital. Instead of taking action, she told us that the family would have to call 911 ourselves. When we called, even the 911 operator questioned why the family was calling from inside a skilled nursing facility and asked if there wasn’t a nurse available to handle the emergency. That alone speaks volumes about the situation we were placed in. We immediately requested an ambulance. Even after EMS arrived, the nurse never returned to the room to check on my uncle or assist with his care. As a family, we were completely shocked by the lack of compassion, urgency, professionalism, and basic patient care. Watching a nurse ignore a patient who was actively having a seizure is something I will never forget. No family should have to experience what we did. The CNAs we encountered were caring and compassionate, and I appreciate the way they treated my uncle. However, the lack of concern and professionalism from the nursing and leadership staff overshadowed that. I strongly urge anyone considering this facility to think twice before trusting them with the care of a loved one.

My brother has been a resident here for over 4 years. We have gone through multiple managerial changes. I must admit that there has been rough times from a communication perspective. However, there are a couple of staff members that help ease my mind when times get rough. Ms. Joyce and Ms. Mohamed have been a GREAT support when things didn’t go as well as intended. There are a few others that I commend as well but I won’t call all of their names. I made an effort to connect with the staff to ensure that my bro is not forgotten and that his care is the best that they can give! Thanks for your…

I visited Lake City Center to spread a little love to a friend for his bday and father's day coming up. As I enter the center the receptionist help me through registration and showed me where I could find my party. The center was clean and my friend was also nice and clean. I found him in a bible service, which I enjoyed my self.i enteracted with some of the other residents too.Keep up the good work Lake City Center.

It was truly a hard decision to place my mother in a nursing home. Lake City has given me a piece of mind. The quality care she receives has put my fears at ease. It was truly a smooth transition. Nurse Wendy Morgan was my mother's first am nurse She is very knowledgeable, hands on, and has a listening ear to my cares and concerns. I know I made the right decision. Thank You Lake City.

My family has had a wonderful experience with this nursing home. The staff consistently demonstrates compassion, professionalism, and genuine care for the residents. It gives our family great peace of mind knowing our loved one is in a safe, supportive, and caring environment. One person who has made a particularly positive impression is the new Director of Nursing, Takimah. She has been incredibly receptive, approachable, and responsive to concerns and questions. It is clear that she values communication with families and is committed to ensuring residents receive high-quality care. Her willingness to listen and follow up has made a noticeable difference and has helped build trust and confidence in the facility. We are grateful for the dedication of the entire team and appreciate the positive atmosphere they create every day. I would highly recommend this nursing home to anyone looking for quality care for their loved one.

This place is the best. My grandma was here and she talks so good about the staff, her favorite was Jason. She told me that he was so sweet and always took great care of her and would take time to talk to her whenever she felt lonely. Would def recommend.

The purpose for which I visited was to minister God’s word of hope and healing to the residents. I was not too satisfied with the organization. I hope that in the feature the residents will be well prepared for the meeting as if they were in their local churches. I was so pleased with the reception from the front desk staff. She did know her job. I was also impressed with the cooperation of the residents that could make it to the meeting. They joined in the hymn we sang before the ministration of the word. They sang happily with joy.
