Terrible place. The care here is they dont care. Staff is incompetent and disrespectful with zero supervision. After 5 you cannot reach anyone and on weekends the same. This facilty has gone through 3 administrators in the past few years that im aware of. The facilty has recently changed ownership from bad to worse. My mother keeps a UTI and a infection of some sort constantly. Medical records from the hospital prove it. The place smells like severe feces and urine. Im moving my Mother asap. The upper management is a complete joke. Of course management changes so frequently its insane. The wrong people in authority will make any business fail. Im a business owner and have been for 28 years and I've never seen such a circus as I have in this place. Do your homework before you consider placing a loved one here. It will be a easy decision once you see for yourself. The staff barely move around but they sure can play around on cell phones we have noticed at the nurses stations. Two Doctors said move my mom because the care is the worse good news is they both put it in writing for us and signed the paperwork. Corporate you really should be ashamed of yourself for allowing this to continue. Someone may take a legal approach if things dont change immediately. What is wrong with you people's minds? Would you put your own loved one in your facilty? I doubt it. Do the right things and right follows. Do the opposite and consequences are definitely coming.

I trusted this facility to take care of my mom while she was there for rehabilitation. I will say that the staff did a good job keeping me updated on her daily progress, and I appreciated the communication. However, that does not change what ultimately happened to her while she was in their care. My mom developed a bedsore that was not properly treated, and it got so bad that it turned into a deep wound, leaving a hole in her tailbone area. Because of this, she developed sepsis, which is a life-threatening infection. Now we’re back home dealing with a medical emergency that never should have…

Now we’re back home dealing with a medical emergency that never should have happened in the first place. Watching my mom go through this has been heartbreaking. We trusted this facility to provide proper care, monitor her condition, and treat her wounds, but instead her health continued to decline under their watch. No family should have to see their loved one suffer like this. While I appreciate that they kept me informed throughout her stay, I cannot overlook the outcome. My mom deserved better care, and our family is now left dealing with the physical, emotional, and medical consequences of what happened. Based on our experience, I would not recommend this facility.

0/0 I have serious concerns about the standard of care at this facility. Staff are instructed to "double brief" residents by placing a pad and a chuck inside one brief, which I believe is not best practice for skin integrity or dignity. Leadership, including Don Preston, prioritizes staff convenience over resident well-being. Preston states they are "protecting patients," but true protection means preventing conditions from worsening. A significant portion of residents here develop pressure injuries/bed sores. This appears related to the facility’s policy of changing incontinent residents…

This appears related to the facility’s policy of changing incontinent residents only twice per shift, which is inadequate for proper skin care and moisture management. I don't recommend this facility to families or to healthcare professionals who prioritize providing high-quality, patient-centered care.

Our family's experience with Brentwood has been very positive so far. From the very beginning, the admissions process was smooth and welcoming thanks to Ms. Treva, the Admissions Director. She was phenomenal-kind, patient, and incredibly helpful in guiding us through each step. Anytime we've had questions or concerns, she has gone above and beyond to make sure we feel informed and supported. Placing a loved one in a facility is never easy, and one thing I've learned is that family involvement is very important. You can't just leave your loved one-you have to stay engaged, visit often, and be present. Like any place, there are stronger and weaker staff members, but overall, we've had a great experience with the team here. We truly appreciate the care, attention, and compassion that many of the staff show to our loved one. It gives us peace of mind knowing they are in a place where people genuinely care.

I would rate this facility below zero if I could. My father passed away in the hospital in February after developing sepsis due to what I believe was neglect of his catheter care at this facility. He was on a feeding tube and became severely dehydrated, which raises serious concerns about whether he was receiving adequate nutrition and hydration. His overall care was deeply troubling. My father was diabetic, yet his skin was not properly maintained his feet were peeling, his lips were extremely chapped to the point of cracking and bleeding, and his face appeared unwashed for days, becoming raw and irritated. No patient should ever be left in that condition. My father passed away on February 20, 2026, and since then, I have not received a single word of condolence from this facility. No one has reached out regarding his belongings or offered any follow-up communication. The lack of compassion and accountability is unacceptable. In addition, the facility itself was consistently unclean and had a strong, unpleasant odor every time I visited. Staff were difficult to reach, and there was little to no follow-through on concerns or requests. I strongly urge others not to place their loved ones in this facility. Based on my experience, the level of care, communication, and professionalism is extremely poor.

I could not believe a veiw that was posted, mention that they would not send a dog to this place, however it's true. They are not helping their patients to get better but get them ready to die. They don't answer the ring side assists bell , don't wash them and don't monitor their dietary schedule. This is like an hospice more than an nursing home. If you wanted to have your love one not to suffer in agony. Take care of them yourself. Help them by loving them and give them your time.

Unsanitary and my mother would have died if we did not request transport to hospital. She had a blockage and they ignored our concerns. Filed a complaint with department of health and insurance.

I have seen to events with this group of people and both where amazing. The staff treated all the residents so good. I was impressed. Also impressed with the amount of love I saw given to each resident. It felt like one big family. So amazing.
